• Medicines/injections required in case of regular dialysis.
• Help for getting regular dialysis if advised.
• Depending on individual cases & funds required, we give financial help-
  • • Prior kidney transplant
    • In the process of kidney transplant

   

• Medicines for patients post transplant.

The documents required for the financial assistance along with the form are-

1.  Aadhar card or any other government issued id card
2. Pan card, if any
3. A recommendation letter from your doctor along with prescriptions(listing medicines,infections etc) , recommended and/or undergoing treatment OR previously had hospital discharge summary in case of hospitalization.
4. BPL card, if any
